REGOLAMENTO ATTIVITÀ VOLI BIPOSTO DI FAMILIARIZZAZIONE 

  1. L'intero ammontare derivante dall'iscrizione alla scuola di volo viene versato all'atto dell'iscrizione stessa. Il candidato al volo di ambientamento in biposto viene indicato da qui come "iscritto".
  2. Anche nel caso in cui l'iscritto rinunci a frequentare l'attività, dovrà pagare la cifra derivante dall'iscrizione.
  3. Per partecipare al volo di ambientamento gli iscritti devono aver consegnato e compilato la relativa richiesta.
  4. Le polizze assicurative "infortuni" non sono fornite dall'A.S. Etna Fly e devono eventualmente essere contratte personalmente dall'iscritto con le compagnie assicuratrici.
  5. L'iscritto ritira l'attrezzatura tecnica per l'esercitazione. È responsabile di questa ed è tenuto a riconsegnarla al termine della attività agli incaricati dell'A.S. Etna Fly. L'attrezzatura consiste nell'imbrago e nel casco.
  6. Smarrimenti, furti, danneggiamenti delle attrezzature prese in consegna comportano il risarcimento completo del danno da parte dell'iscritto.
  7. L'iscritto deve utilizzare calzature personali adatte all'attività di volo libero. Questa comporta un decollo ed un atterraggio effettuati di corsa su terreni che possono essere sconnessi. Si consiglia l'uso di calzature per il trekking. La scelta delle calzature utilizzate resta comunque sotto la personale responsabilità dell'iscritto.
  8. L'abbigliamento deve consentire di poter affrontare l'abbassamento di temperatura che un eventuale guadagno di quota può comportare. Comunque è sconsigliato volare a braccia o gambe scoperte. Si suggerisce l'utilizzo di guanti e occhiali sportivi.
  9. È vietato partecipare con orecchini, bracciali, anelli, occhiali da sole con montature metalliche o con lenti non infrangibili e con tutti gli oggetti che possono rappresentare un possibile rischio per l'iscritto.
  10. Gli iscritti devono tenere durante le esercitazioni un comportamento prudente e responsabile attenendosi scrupolosamente alle direttive del personale dell'A.S. Etna Fly.
  11. L'iscritto deve essere a conoscenza delle nozioni che gli consentiranno di collaborare attivamente col pilota. Tali informazioni essenziali per la corretta esecuzione dell'attività vengono date all'iscritto nel briefing antecedente alla attività di volo dal responsabile dell'A.S. Etna Fly. L'iscritto deve essere consapevole che solo attenendosi a queste precise indicazioni è possibile svolgere correttamente l'attività di volo. L'iscritto prima di iniziare l'attività di volo deve essere certo di aver compreso chiaramente le nozioni impartite dal responsabile dell'A.S. Etna Fly. Il volo in biposto comporta dei gravi rischi nel caso in cui l'iscritto non dovesse eseguire correttamente le procedure di decollo, di volo e di atterraggio.
  12. L'iscritto usufruisce gratuitamente delle attrezzature tecniche.

DICHIARAZIONE DI PARTECIPAZIONE BRIEFING PRE-VOLO

Il trasportato dichiara di aver partecipato al long briefing pre-volo (relazione tecnica esplicativa) apprendendo le tecniche che gli consentiranno di collaborare attivamente col pilota per la corretta conduzione del volo. Specificatamente il "long-briefing pre-volo" ha trattato i seguenti argomenti che sono stati tutti chiaramente compresi:

  1. Descrizione sommaria del tipo di volo che si intende effettuare.
  2. Vestizione e corretto uso attrezzature tecniche.
  3. Controlli personali su abbigliamento, accessori e oggetti da portare in volo.
  4. Regolazione dell'imbraco e prova imbraco simulata.
  5. Comportamento durante fase di gonfiaggio (solo parapendio).
  6. Comportamento durante corsa di decollo.
  7. Tecniche per la sistemazione nell'imbraco in volo.
  8. Corretta posizione e comportamento da tenere in volo.
  9. Comunicazioni in volo.
  10. Gestione di eventuali emergenze.
  11. Preparazione all'atterraggio.
  12. Corsa di atterraggio.

Il trasportato è consapevole che solo attenendosi a tutte le precise norme è possibile svolgere correttamente l'attività di volo. Il trasportato è certo di aver compreso chiaramente tutte le nozioni impartite. Il trasportato è consapevole che il volo in biposto comporta dei gravi rischi nel caso in cui il passeggero non esegua correttamente le procedure previste per decollo, volo ed atterraggio.

DICHIARAZIONE DI AVVENUTA ATTIVITÀ

Il trasportato avendo effettuato con successo l'attività di volo con la presente dichiara di non aver subito alcun inconveniente durante l'esecuzione della stessa.

LIBERATORIA E CONSENSO ALLA PUBBLICAZIONE E TRASMISSIONE DI IMMAGINI

Il sottoscritto autorizza a titolo gratuito e senza limiti di tempo, anche ai sensi degli artt. 10 e 320 cod.civ. e degli artt. 96 e 97 legge 22.4.1941, n. 633 – Legge sul diritto d'autore: la trasmissione e la pubblicazione e/o diffusione in qualsiasi forma di materiale audio, video o fotografico in cui il sottoscritto appaia rappresentato o sia comunque riconoscibile. Il materiale audio-video-fotografico che pregiudichi la dignità o il decoro delle persone interessate non sarà oggetto di trattamento e sarà immediatamente cancellato dagli Archivi dell'A.S. Etna Fly. La pubblicazione/diffusione potrà avvenire sui siti internet dell'A.S. Etna Fly, sui canali social dell'A.S. Etna Fly (Facebook, Youtube, Instagram, Twitter ecc.), su carta stampata e/o su qualsiasi altro mezzo di diffusione/pubblicazione anche atipico. Autorizzo altresì la conservazione del predetto materiale negli archivi informatici dell'associazione e prendo atto che la finalità di tali pubblicazioni sono di carattere informativo e divulgativo. Il materiale video-fotografico non sarà utilizzato per finalità commerciali. Tale autorizzazione esclude qualsiasi uso a fini di lucro e la cessione a terzi. In considerazione della circostanza che il predetto materiale, una volta reso pubblico, può essere oggetto di acquisizione e di ripubblicazione da parte di terzi, anche senza il consenso dell'A.S. Etna Fly o dell'interessato, sollevo l'A.S. Etna Fly da ogni effetto pregiudizievole che possa derivare da un uso abusivo o scorretto da parte di terzi del predetto materiale audio-foto-video. Confermo di non aver nulla a pretendere in ragione di quanto sopra indicato e di rinunciare irrevocabilmente ad ogni diritto, azione o pretesa derivante da quanto sopra autorizzato.

REGULATIONS FOR TANDEM FLIGHT FAMILIARIZATION ACTIVITIES

  1. The entire amount due for the flight school registration must be paid at the time of registration. The candidate for the familiarization tandem flight is hereby referred to as "enrolled".
  2. If the enrolled decides to withdraw from the activity, they must still pay the amount due for the registration.
  3. To participate in the familiarization flight, the enrolled must have submitted and filled out the relevant request.
  4. "Accident" insurance policies are not provided by A.S. Etna Fly and must be personally arranged by the enrolled with insurance companies if necessary.
  5. The enrolled collects the technical equipment for the exercise. They are responsible for it and must return it at the end of the activity to the appointed staff of A.S. Etna Fly. The equipment consists of the harness and helmet.
  6. Losses, thefts, damages to the equipment taken into custody entail the full reimbursement of the damage by the enrolled.
  7. The enrolled must use personal footwear suitable for free flight activity. This involves a take-off and a landing performed by running on possibly uneven terrains. The use of trekking shoes is recommended. The choice of footwear used remains the personal responsibility of the enrolled.
  8. The clothing must allow facing the temperature drop that a possible gain in altitude may involve. However, flying with exposed arms or legs is discouraged. The use of gloves and sports glasses is suggested.
  9. It is forbidden to participate with earrings, bracelets, rings, sunglasses with metal frames or with non-shatterproof lenses, and all objects that may represent a possible risk for the enrolled.
  10. The enrolled must maintain a prudent and responsible behavior during the exercises, strictly adhering to the directives of the A.S. Etna Fly staff.
  11. The enrolled must be aware of the notions that will allow them to actively collaborate with the pilot. Such essential information for the correct execution of the activity is given to the enrolled in the briefing preceding the flight activity by the responsible of A.S. Etna Fly. The enrolled must be aware that only by adhering to these precise instructions it is possible to correctly carry out the flight activity. The enrolled must be sure to have clearly understood the notions imparted by the responsible of A.S. Etna Fly before starting the flight activity. Tandem flying involves serious risks if the enrolled does not correctly follow the take-off, flight, and landing procedures.
  12. The enrolled benefits from the technical equipment free of charge.

STATEMENT OF PARTICIPATION IN PRE-FLIGHT BRIEFING

The transported declares to have participated in the long pre-flight briefing (technical explanatory report), learning the techniques that will allow active collaboration with the pilot for the correct conduct of the flight. Specifically, the "long pre-flight briefing" covered the following topics, all of which were clearly understood:

  1. Brief description of the type of flight to be performed.
  2. Dressing and correct use of technical equipment.
  3. Personal checks on clothing, accessories, and objects to bring on the flight.
  4. Adjustment of the harness and simulated harness test.
  5. Behavior during the inflation phase (paragliding only).
  6. Behavior during take-off run.
  7. Techniques for settling into the harness in flight.
  8. Correct position and behavior to maintain during flight.
  9. Communications during flight.
  10. Management of potential emergencies.
  11. Preparation for landing.
  12. Landing run.

The transported is aware that only by adhering to all the precise rules is it possible to correctly perform the flight activity. The transported is certain to have clearly understood all the notions imparted. The transported is aware that tandem flying involves serious risks if the passenger does not correctly execute the procedures provided for takeoff, flight, and landing.

STATEMENT OF COMPLETED ACTIVITY

The transported, having successfully carried out the flight activity, hereby declares not to have suffered any inconvenience during its execution.

RELEASE AND CONSENT FOR THE PUBLICATION AND TRANSMISSION OF IMAGES

The undersigned, authorize free of charge and without time limits, also pursuant to articles 10 and 320 of the civil code and articles 96 and 97 of law 22.4.1941, n. 633 – Copyright Law: the transmission and the publication and/or dissemination in any form of audio, video, or photographic material in which the undersigned appears or is otherwise recognizable. Audio-video-photographic material that prejudices the dignity or decorum of the interested parties will not be processed and will be immediately deleted from the Archives of A.S. Etna Fly. The publication/dissemination may occur on the websites of A.S. Etna Fly, on the social channels of A.S. Etna Fly (Facebook, Youtube, Instagram, Twitter, etc.), on printed paper and/or on any other means of dissemination/publication even atypical. I also authorize the storage of the said material in the digital archives of the association and I am aware that the purpose of such publications is informative and educational. The video-photographic material will not be used for commercial purposes. This authorization excludes any use for profit and transfer to third parties. Considering that the said material, once made public, can be subject to acquisition and republication by third parties, even without the consent of A.S. Etna Fly or the interested party, I relieve A.S. Etna Fly from any prejudicial effect that may arise from abusive or incorrect use by third parties of the said audio-photo-video material. I confirm that I have nothing to claim in reason of the above and irrevocably renounce any right, action, or claim arising from what has been authorized above.
